Futbol Club of Santa Rosa 2011-2012 AGREEMENTTO PAY PLAYER FEES
SELECT PROGRAM
1. I agree to pay the full amount of the annual player fees for my child to participate as a member of the FCSR team for which he/she is selected. recognize that by agreeing to be a member of the team, my Child is taking a position that may not be filled aher the start of the year, and the team is relying on my payment to be able to operate. Therefore, even if my child elects to leave the team during the 2011-2012 Season, I agree that the full amount of the fees is payable by me. No player will be released from our club to join another club if all fees have not been paid and will be put in bad standing with FYSA.
2. In the following two situations, an exception to #1 may be requested and seasonal fees may be pro-rate based on the date of departure during the season. All such requests must be formally submitted to the FCSR governing Board for their approval.
- My child incurs a season ending injury confirmed by medical documentation .
- My child moves beyond a reasonable travel distance. Determination of a reasonable travel distance is at the sole discretion of FCSR governing Board.
3. I agree that Club player fees are non-refundable. If my child’s team folds, disbands for any reason, during the soccer year, August 1, 2011 through May 31,2012, the Club is not required to refund any money.
4. I agree to pay my share of additional assessments made by the team or the Club during the year to cover all team expenses, such as coach’s travel costs, costs for tournaments, league fees, etc. These costs are not covered by the club.
5. I understand that the player fees do not include the cost of the required uniform and that each player is responsible for their own uniform.
6. If my check or checks are returned for insufficient funds/stop payment, I agree to pay an additional $30.00 per returned/stopped payment check.
7. I agree to pay a $5 late fee for each payment thatis not received by the io" of the month.
8. I will pay the annual player fees as follows in the schedule below. Play level is determined by your coach at tryouts. PLEASE INITIAL ONLY THE PAYMENT OPTION SELECTED.

10 month training program
Option A:
Payment in full $850.00 due upon registration.
Option B:
$100.00 registration fee and
$375.00 due upon registration
$375.00 due 1/1/12
Option C:
$100.00 registration fee plus 1st monthly installment ($175.00) due upon registration.
$75.00 to be paid on 1st of each month: Sept. 1, Oct. 1, Nov. 1, Dec. 1, Jan. 1, Feb. 1, Mar. 1, Apr. 1, May 1

Option A:
Payment in full: $700.00 due upon registration.
Option B:
$100.00 due at registration
$200.00 due July 1
$200.00 due Sept. 1
$200.00 due Oct. 1
*all fees for high school players must be paid by Oct. 1
*if training for a tournament during high school season, an additional monthly training fee will apply
